Adult Orthodontics: MARPE, Invisalign, and Digital Planning with Dr. Hwang

Case #1) In the first case presented, Dr. Hwang addressed a 43-year-old patient with a narrow palate, neck pain, and headaches. The patient had prior premolar extractions, and he demonstrated his MARPE protocol, including palatal hooks and careful integration with Invisalign. Using Itero scans and software simulations, he planned controlled 5 mm expansion while optimizing skeletal and dental outcomes, showing how digital planning allowed precise measurement of intercanine, premolar, and molar width.
Case #2) The second case focused on a patient with a collapsed upper midline following premolar extraction. Dr. Hwang discussed anchorage planning, the use of springs for efficient canine mesialization, and strategies for managing complex space closure. Through software visualization, he evaluated the occlusal plane and tooth movements, ensuring predictable results and minimizing complications.
Case #3) In the third case, he evaluated a patient with severe bilateral posterior crossbite and mild sleep apnea. He emphasized skeletal expansion, mid-facial deficiency correction, and the use of soldered hooks for face mask elastics. Software tools helped him assess skeletal discrepancies, simulate expansion outcomes, and coordinate MARPE, elastics, and Invisalign to address both functional and esthetic concerns, while carefully considering the smile arc and facial profile.
Across all cases, Dr. Hwang highlighted how digital software enhanced treatment planning, allowing precise measurements, accurate simulation of outcomes, and improved predictability for complex adult orthodontic patients.
